New Concept Care & Nursing
has experience of LD provision will use
a team recruited for their experience in looking after the specific needs
of a particular service user and will be led by a Registered Learning
Disability Nurse. It is essential the service user is comfortable
and confident with the carers, so they will always have an input in this
process.The support will often be 1-1 to to enable the them to work towards
reducing their behaviour problems by building on their strengths and developing
their range of useful skills and abilities. This could be achieved through
access to community facilities for Support can also be provided in a service user’s own home, either to enable respite for full time carers, to support an existing carer to promote independence and well being in the service user. this could vary from an hour or two a day up to periods of 24 hour care. Initial assessment, working in conjunction
with Community LD nurses, psychology and key workers from other services,
will produce a care plan, risk assessments and behaviour/epilepsy management
plan as relevant. These are linked to person centred planning and incorporate
O’Brien’s five accomplishments Ongoing care management is provided by the team. The Care Plan is thus reviewed as the demands of the service user change and everybody in an LD team will have have ongoing training and support relevant to these changing needs. We are
