Independent Supported Living

New Concept Care & Nursing can help ameliorate the circumstances that can make the move from living in residential care or at home with a family to an independent supported living (ISL) environment a difficult step. We have proven experience of working with other community agencies to provide support to allow someone aspiring to such an objective to succeed. We can
- Arrange accommodation
- Ensure it is furnished to a suitable standard
- Monitor that the level of risk is acceptable.
- Organise the environment to provide adequate privacy

We will the ensure that it functions as a happy, supportive environment by
- ensuring it is maintained adequately so it is always a safe and friendly environment
- monitoring the interactions between residents and neighbours
- helping residents to budget and control their finances. This will include support to pay
- bills as well as claiming and collecting benefit

We realise that LD service users, especially those with challenging behaviour, will change and grow and that their needs will change and grow in consequence. Our multi disiplinary support team will meet regularly with the service user and others who support them to ensure that their care plan is constantly monitored and is then modified as necesary to reflect this.
